I hereby grant the DevxSoftware Inc consent to process my personal data in the form of acquisition, collection, storage for the purpose of executing a contract and/or the intention to enter into a contract, to the extent of: name, surname, date of birth, residential address, passport/ID-card data, e-mail contact, phone contact, bank details, contact details for emergencies.

This consent to the processing of personal data is a freely given, specific, informed and unambiguous expression of my will. Prior to giving consent, I was informed of the following:

• my personal data, the provision of which is voluntary, will not be processed for any purpose other than that for which they were obtained;

• as a data subject, I have the right to request from the DevxSoftware Inc access to my personal data concerning me, the right to be informed, the right of access, the right to rectification, the right to erasure, the right to restrict processing, the right to data portability, the right to object, as well as rights in relations to automated decision making and profiling, right to compensation;

• I have the right to lodge a complaint with a supervisory authority;

• I have the right to withdraw consent to the processing of personal data at any time by e-mail sent to the address hr@developex.com.

Withdrawal of consent does not affect the lawfulness of the processing of personal data based on consent present prior to its withdrawal. The data subject is informed of the above before giving their consent.
